im at a loss of words for your passing. i would never be the baseball player i am today if it wasnt for you. i still remember the day you gave me my nick name "izzy" you told me i play pretty so you combined "isaiah" and "izzy" together its amazing how you gave me that nickname when i was 11 and its still stuck with me to this day me being 17. that just goes to show you the legacy you left behind and the impact you had on peoples lives. however far my life takes me or however far sports take me i will always owe it to you. you taught me through baseball the most valuable life lesson...when you cant do something and it causes you to fall to the ground, get up until you can do it. you taught me how to persevere how to know what it feels like to fail and the satisfaction of accomplishing something through hard work. ill never forget the lessons you taught me and the smiles you gave me in the time i played under your coaching. just know, every time i put on a glove or strap on a cleat you will be on my heart or anytime i feel tired and need to take a break i will imagine your voice screaming at me like old times forcing me to go on. your life here on earth might of passed but your legacy will forever live on through the lives of the people who had the blessing of knowing you. I was very sad to hear the news of Sam's passing, I worked with Sam for 7 years at Health Net in shelton, CT. Although I knew him only through passing Sam has a big spiritual presence. I always admired him for his work ethic, he was always helpful and went the extra mile to ensure the job was done. By his demeanor I can tell that he loved his family and received that love back ten times fold. My heart,thoughts and prayers go out to his family, though he is not here physically his memories will keep his spirit alive. Gone but not forgotten, He is now at peace by the lord's side.

I not only had the honor of growing up with Sammy, I had the priviledge of having him coach my son Robbie in baseball "YANKEES" at North End Little League, which made it extra special to me..Sammy was always kind, always smiling,always had something positive to say..He will be missed dearly... My dearest Sammy, I'm sitting here trying to remember when we first met but I can't pinpoint a date/time - you've been a part of my life as far back as I can remember - heck some of your cousins are my cousins :-) When I first heard that God had called you home I didn't want to believe it, praying it was a bad dream. I had just spoken to you over a month ago. But then reality hit and memories came flooding back, to name a few - visiting my cousins on Laurel CT and playing out on the street with everyone, then when we were in youth group at Good Shepherd - hanging out at Beverly's after church on Friday nights - singing with Voice of Calvary - there are so many memories. One I will hold close to my heart is when God called mom home you called me a few days before that and first thing you said was "Hey Chiqui mom said you needed a friend" that call meant more to me than you'll know. I will miss our talks where you would talk on and on about Sammy, Matthew & Jeremy and then when you were blessed to be a grandpa, so much love you had for each of them. Sammy, you will always hold a very special place in my heart. You were a true friend to so many people and will be missed so much. Give my mom and your mom one of your bear hugs and tell them I miss & love them. Hno. Sammy was a very special and wonderful friend to our family. Everytime we would see him, he always had that unforgetable smile and we always felt his love when he gave us his Big Bear Hugs. We have know the Rivera/Montalvo family for over 50 years. Neville went to Sammy Sr. and Benny's wedding and he has fond memories of that day..I remember Benny being sooo overdue and Sammy decided to delay his birthdate. I also remember a young Sammy giving his heart to the Lord at many alter calls because he loved the Lord sooo much he just wanted to keep giving his beautiful heart to the Lord over and over again. It was my pleasure to have both Sammy and Bobby work and help me in Vacation Bible School at Good Shepherd.Our deepest sympathy to the entire Montalvo/Rivera Family. God Bless you.We love you. My deepest sympathy to the entire Montalvo family. Sam was certainly a special man and he will always be remembered for being the smiling face at Healthnet security. I will never forget the kindness he showed to me, particularly one day about 10 years ago. I was leaving the HN building during an ice storm one evening and someone came around the corner and hit my car. As I exited my car, another vehicle came around the corner and hit my car again. Sam calmed me down and stayed with me until the police arrived. That was just the kind of man he was. He will be deeply missed.

This one is for You Sammy lets go down memory lane I meet You 43 yrs ago at children church " Good Shepherd " You'er truly not like all the other boys Your Laughter may me feel happy there. Now today "Hearts are sadden , Smile turn into frown, Laughter into tears, a Good sense of Humor into a broken Heart as I have to say Good-bye. I must tell some story's Your HUGS were the Best every time the Smile the Biggest Your Laughter the best You made the room Fun. Also all the Party's , Picinces, Christmas Plays and all the Christmas Paraganda around town great. Especially when Your Mama's home was the last one the she would be waiting with open arms The Gang of Good Shepherd we would even fall asleep all around the house does were the Good Old Days. Also going over just to watch games Football, Baseball ect. on the TV or just play games it was all so much FUN. Even when we all stay at the church because Our parents were making Pasteles to sale for the church. All we did was play and then find some of the kid's sleeping around on the floor. But must of all seeing You grow up in the Lord and be come Our Youth Pres. and a leader. You turn from a boy, Teen, Man, Dad, and also a Grandfather I feel so Honor to call You my brother and to known You has made a big defences in my live. Or the time that I felt the worse in my live was when my Dad went Home to be with the Lord You My Dear Friend came and gave me the Greatest Biggest HUG ever and with your smile made me smile does are some of the thinks that I can not forget. You are so special to me. Hey I'm still trying to say Good-bye but more story all the Camping with so many more Youth and learning about the Lord along the way the best, Trips around the New England area awesome, Birthday Party's like sweet 15 or 16 with The Gang Fun, Picincs at Sherwood Island Park play baseball or touch football because the girls were playing. Then it was swimming time what FUN we had. Even at Lakequacesa Park where we had rides to get on, games, food all day and more Fun all though out the day seeing You smile & hearing You laughter. Now New York for the Christian Parades and walk for 5 miles long we also did the with so many people and we let them know that there Is a God. Now this is the last one Pizza Party after the Youth Service with The Gang and Your sense of Humor so much FUN all the laughing going on. Or over the Adorno in the summer time for swimming rigth after school was done for the yr.and on Sunday after church and before the night service on Sunday I'm telling You we had so much Fun we didn't have PC just each other has a Big Happy Family in the Lord which made Us brothers & sister for life. As we got older it was easy to catch up with life because we know each other and kept in touch every couple of yrs personally or though Mama Benny if I didn't see You when I went back Home. Brother this is not easy words can't express who You are here are some Strong, Loving, Cheerful, Compassionate, Respectful, Caring, Sharing, but most of All a True Friend. You have left a impacted on my life and so many other as well. My Heart is broken in knowing that I will not see You here no more but just knowing that I will see You SMILELLY Face in Heaven when I get where one day soon makes me smile. Thank You for sharing Your Life with me, Love YOU Big Guy missing You already. Now come down and give me a HUG so I can stop Crying. You will never be forgotten You have inpacted to many Life. I first met Sam when I was hired by HealthNet in December 2002 as a Corporate Security Supervisor. It was only a few weeks of working together before I realized that Sam was a valuable, loyal, and dedicated associate, truly my right hand man. I never hesitated to travel anywhere the company required me to because I knew that Sam could handle running the office in my absence. During our eight years together Sam really embraced the training offered, and loved being able to lead and organize valuable security and safety training which would benefit everyone. In the last few days I have been contacted by many of Sam's friends and associates from across the country. We will all miss him dearly!

When I worked at HealthNet back in 05-06 Sam was my boss. He helped create a great working environment and was very helpful to the new guy (me) getting comfortable coming to a new site. So many amazing words come to mind when i think of Sam. Funny, loving, sweet, generous, strong.. just to name a few. He welcomed me into his family and took care of me as if i was his own child. Sam was like a second father to me. He was always there when i called & never turned me away. He loved his family, and was always planning something so we can all be together. One thing i know for sure is how much he adored his Granddaughter Sophia. Man he thought the world of her. Sophia loved her Grandpa Sam, and she will grow up with his memory in her heart. I am so sorry to hear of Sam's sudden passing. He was always a professional at Health Net, taking his job responsibilities extremely seriously, always keeping all the associates safe; however, he also loved having a good laugh with the guys when he wasn't being so serious about his job. Dear Lourdes and Family, I’m so very sorry for your loss. My thoughts and prayers go out, especially to your family, and to his friends and co-workers. Sam has passed on to a better place and is with his family that has gone before him. I'm asking why so soon, God? He had so much more to live and to give to all of us who were BLESSED to know. We all have our memories of him that will comfort us. Sam was a WONDERFUL man, had a BEAUTIFUL personality, a GREAT sense of humor, and a SMILE that lite up the world. He was the BEST supervisor I have ever worked for. And he was a friend too. He shared many stories with me about his life and I shared stories with him about my life. It turns out that my first kiss as a kid was with his cousin. We are both from Bridgeport and went to Maplewood School and to Bassick High School, but not at the same time as I'm a few years older than him. He went to Southern Connecticut State University for undergraduate school and I went there for graduate school at the same time. What a small world. I wasn't supposed to work on Friday, but he asked me to fill in for another employee. I'm glad I was there and I got to talk with him before he passed on. He often spoke of how much he loved his wife, children and granddaughter. He was so very proud of all of them. I will miss not seeing and talking with everyday at work, but I know he will be there in spirit. May God comfort all of you in your time of sorrow and keep you close to him. To My Brother and Friend; Sammy I am going to miss you very much. Although we have been living away from each other for so many years we always had a very special bond. I have such fond memories of our growing up together I could not have asked for a better brother. I remember the many times we would have our WWF wrestling matches at the house on Wake St. We broke the legs of the couch one night while mom and dad were at church, another night I remember when you hip tossed me and my foot went into the floor fan. Mom and dad were really mad about the broken fan more than my bleeding foot. I remember you standing up with me during rough times and also letting me get my butt kicked when I was in the wrong and deserved it. I remember the time we fought when you moved my precious Buick Skylark in the drive way and hit the house on Garfield Ave creasing the passenger door. But my dear brother, most of all I will always remember your big heart. Your giving spirit, your unselfishness and your big smile. Any accomplishments I had in sports I got from your example of hard work and sacrifice. Let me tell you it was not easy living in the shadow of such a good athlete. You demonstrated to me that the team came first before personal achievements. You impacted many kids coaching at the North End Little League. Your passion for the game and making sure that the kids had fun and learned was second to none. I really enjoyed going to watch the games when I would be home on leave from the Navy. You have some wonderful boys that are proud to call you Dad and a granddaughter that will miss out on the love you have for her. Sam, Although we didn't see each other much, it was always nice seeing you at family functions - you will be missed. It seems I've known you virtually my entire life. I remember you as a little boy at Maplewood School...and when I pull out my class pictures to reminisce, I see your smiling face and will continue to see your smiling face. Dad there are no words that I have that can explain what you mean to me. My world and my life have been flipped upside down. But the love I have for you and how proud I am that I get to say you are my father gets me through. The love and the support you have given to me throughout my life has molded me into the man I am today dad. I know god has a plan for us all and he needed you know but I want to be selfish and just keep you around a little longer. You mean the world to me dad and all I have ever wanted was to make you proud. So you can be the father to sit back and say hey that's my son. My dear friend Sammy, words cannot express how much my heart is heavy since I heard the news of your passing. You were such a wonderful friend..a brother. Within the last 24 hours, my mind has been taking a journey back to the wonderful memories of growing up together at Good Shepherd church. The good times we had in youth group and going to eat pizza with the gang afterwards at Luigi's. I know that you are in a better place now, but you will be missed so much here on earth. I know that you are rejoicing with your savior and happy that you are once again with your mom! To my dear second father, Hno. Montalvo...my prayers are with you as you go through another time of mourning and having to say "see you later" to a loved one. My heart goes out to you. I love you and am praying for you so much!!! To Bobby, I'm so sorry that you lost your big brother. You heart still as not healed from burying your mom, now you have to do the same for Sam. Just remember, God is the one that is holding you in His arms and giving you strength. I love you. To my dear wonderful and precious sister, Liz..oh how my heart grieves for you and your family. You have lost your mom and your big brother. No one should go through that. I just want to let you know, I'm here for, my prayers are inundating heaven, asking God to give you strength and peace. I want to extend my deepest sympathies and prayers to all of Sams's family, on his untimely passing on Friday. It's hard to understand why the Lord takes such a nice person with a fine, full life but I know He has a plan for Sam and will keep him by His side always. Sam was such a wonderful person to work with. He always had a nice smile and was so supportive and helpful to everyone. He'll be sorely missed by all who worked with or knew him. My condolences to the Montalvo family for the passing of Sammy. The family is in our prayers for God's comfort and peace. My heart goes out to each one of you - Sammy was also part of my family! What a wonderful person Sammy was! Always pleasant and would always give nice warm hugs!! A teddy bear!! At my wedding Sammy was my ringbearer and as we took pictures I remember Sammy running at the park with my dear cousins Aileen and Mickey who were also part of my wedding party. It seems it was only yesterday!! We can't understand God's plans for us but we understand his plans are perfect!!! May the great memories of good times console you through this difficult time. Sammy has come to the end of his days here on earth but continues to live in a wonderful place called by Jesus His father's house!!
